I, the undersigned parent/guardian, hereby permit for my child to participate in the Art Handicraft Workshop hosted by Creative Playzone. I understand that this is a drop-off program, and I will not be staying on-site during the session.
I acknowledge and agree to the following:
- I understand that while all reasonable precautions will be taken, Creative Playzone and its staff will not be held responsible for any injury, loss, or damage during the activity.
- I confirm that my child is between the ages of 4 and 12 and is toilet-trained.
- I will inform the staff in advance of any medical conditions, allergies, or special needs my child may have.
- I authorize the staff to administer basic first aid and contact emergency services if needed.
- I agree to provide accurate and up-to-date emergency contact information and remain reachable at all times during the workshop.
- I agree to pick up my child on time. A late pick-up fee of $5 will be charged for every 10 minutes after the workshop ends.
- If I need to cancel the registration, I will provide at least 48 hours' notice:
- Paid workshops: Fees are non-refundable but may be rescheduled.
- Free workshops: No-shows or cancellations without notice will result in the session being forfeited.
- I agree that photos or videos taken during the workshop may be used for promotional purposes, unless I notify staff otherwise in writing.
